A New Way To Do Memory Care

The vision for Remember Me began in 2014 as Tracy and Andy Sneed saw the need first-hand as their mother, Anne, lived with dementia. Over her 13-year journey with dementia, Tracy and Andy recognized that there was a great need for care, services, and education about dementia. Because of their journey, developing Remember Me was a personal and purposeful way to change the way people perceive both assisted living and dementia care. In 2018, Tracy and Andy partnered with DKLEVY Architecture and Design, Diaz Fritz General Contractors, and Solinity Management to bring the five farmhouse neighborhood that provide personalized care to residents. Since then, Remember Me has revolutionized memory care in Cleveland, TN.

Farmhouse Living 

The Remember Me neighborhood is just that, a neighborhood. The beautiful farmhouses create a community to help residents live their best lives in a safe and familiar home-style environment. Each farmhouse has 12 private bedrooms with their own bathroom, a large, shared living space, and a residential-style kitchen. A member of the dietary team prepares meals in each farmhouse that are enjoyed at a table with other residents, creating olfactory stimulation. There are 12-14 residents per farmhouse, creating a close, family-style environment. 


People with Alzheimer’s disease and other dementias can easily be overwhelmed, confused and/or distracted when faced with large groups or spaces. This applies to activity participation, meal times, and even residential living arrangements. Residents often function better in quieter, smaller environments like in our farmhouse dining rooms. Remember Me Senior Care is a neighborhood community unlike any other senior living option, specifically created to let our memory care and assisted living residents thrive and feel at home, which is a very unique approach from traditional nursing homes.

Consistent Social Care

The design of the Remember Me neighborhood promotes social interaction, allowing residents and families to engage as they would in a community setting. This can be attributed to the sidewalks, green spaces, courtyards, and centrally located gazebo in the neighborhood. The five farmhouses are organized within a familiar neighborhood cove-like layout, featuring welcoming front porches with rocking chairs and seasonal landscaping. This familiar and comfortable setting helps ease residents and enables them to enjoy their surroundings. 

This is further enhanced by our resident-centered care and consistent staffing, aimed at fostering personal relationships among residents, their families, and our team members. This dedication to the holistic care of residents can profoundly impact residents with dementia through prioritizing their emotional, physical health, and general well-being. 

With a friendly setting and a focus on social interaction, Remember Me strengthens relationships through group activities. Companionship takes various forms, especially through our personalized and individualized daily activities tailored to residents’ interests and cognitive abilities. With a constantly evolving activities calendar, there's always plenty to keep residents engaged and entertained.

Remember Me has a low caregiver-to-resident ratio, enabling staff to cultivate meaningful relationships while providing personalized and intimate care to each resident. With 24/7 around-the-clock care, families can rest assured, regain confidence, and enjoy visiting again, knowing their loved one is receiving the tailored care they deserve. 

We also prioritize the well-being of our residents by offering comprehensive memory care training for our team. Our training, through Rachel Wonderlin's Dementia By Day program, emphasizes understanding various forms of dementia, effective communication techniques, and person-centered care approaches. Through ongoing education, our dedicated staff members are equipped to create a supportive and enriching environment for residents needing memory care, fostering a compassionate and nurturing community at Remember Me.
